I am a volunteer with the Prior Lake Waterski Association (PLWSA), also known as the "Shakopee Prior Lake Waterski Association." Volunteering with the SPLWSA enables me privileges to visit, watch, and/or participate in association waterski activities, some of which may involve the risk of bodily injury or property loss. With full knowledge and acceptance of the risks of doing so, I hereby for myself, my heirs, executors and administrators, release and forever discharge the SPLWSA, its officers, directors, members, and representatives, of and from any and all, claims, demands, and actions whatsoever that I may have for any and all loss, damage, or injury sustained by me during any such activities.

 

I acknowledge that Quarry Lake property is owned by the City of Shakopee (City) and is made available to and used by the certain members and guests of SPLWSA under a Use Agreement between the City and SPLWSA which defines SPLWSA Member rights and obligations at Quarry Lake. In return for these Quarry Lake access and use privileges, I hereby for myself, my heirs, executors and administrators, release and forever discharge the City of Shakopee and SPLWSA, their officers, directors, members, employees, and representatives of and from any and all claims, demands, and actions whatsoever that I may have for any and all loss, damage, or injury sustained by me in connection with such access or use. 

 

By checking this box you or designated parent or guardian agree to the above LIABILITY RELEASE, WAIVER AND AGREEMENT. This extends to American Water Ski Association, Minnesota Water Ski Association, USA Waterski & Wake Sports, the City of Shakopee, the City of Prior Lake, SPLWSA, Shockwaves Adaptive Waterski Squad, the SPLWSA Board of Directors and SPLWSA Program Directors. I/We have read, understand and agree to comply with the Release, Waiver and Agreement as outlined above.

By participating in the Shakopee Prior Lake Water Ski Association/Shockwaves Adaptive Ski Squad Clinic and entering the Quarry Lake premises, you automatically agree to be filmed and/or photographed for any and all media purposes known now or to be developed, into perpetuity, without compensation or acknowledgement.
